Annual Block Fee is available at our Bristol Office location.
The Block Fee will cover unlimited access to certain services not covered by OHIP.
This is valid until the end of the calendar year in which the Block Fee was purchased.
The coverage does not extend for 12 months from purchase into the following calendar year. Patients have the option to pay per service as listed on this website.
The following services are covered by the Block Fee:
- Sick Note for school/work
- Notes for Third Party (e.g. Physio, Massage, Chiro)
- Notes for Custom Products (e.g. Orthotics, Shoes, Stockings)
- TB skin Tests (including note)
- Phone/Fax or email Prescription Renewals
(Prescription renewals are non-Narcotic and based on Physician discretion)
The annual cost of the Block Fee is $99.
This is an introductory price and may change at any time at the discretion of management. A price increase may not be reflected on this website so please call or drop by the office to confirm the current rate. Patients have the option of reimbursement within 7 days of purchase but any services rendered would be charged on a per service basis and this fee deducted from the reimbursement amount.
